MINIMUM QUALIFICATIONS

§ Must be 21 years of age or older

§ Minimum 16 hours of yearly training

§ First Aid certified

§ CPR certified

§ Must be able to read and write English

§ Excellent interpersonal skills

§ Minimum degree in related field as based on DBHDD standards

§ Clean driving record

§ Clean Criminal Record Check

DUTIES AND RESPONSIBILITIES

· Train the staff on how to best meet the resident’s needs

· Conduct staff trainings

· Provide information on the best way to serve the individual based on the disability

· Supervise ‘at-risk” individuals

· Oversee the services and supports to the individuals served by the agency

· Overseeing the services and supports provided to participants that include;

· Monitors and/or participates in the implementation and delivery of the Individual Service Plan (ISP).

· Supervises the delivery of service and ensures the strategies reflect the goals and objectives of the ISP.

· DDP monitors the progress toward achievement of goals in the ISP, and makes recommendations for modifications to the ISP, as appropriate.

· Supervising the formulation of the participant’s plan for delivery of all waiver services provided to the participant by the provider, on an annual basis subsequent to ISP development and after any ISP addendum that includes, but is not limited to;

· Ensuring the implementation strategies reflect the ISP and the needs of the participant

· participates in the development of the ISP

· Supervising high intensity services that address health and safety risks for the participants that include.

a. The agency DDP is involved in reviewing and/or writing, and the implementation and effectiveness of the Crisis Plan

b. The agency DDP is involved in identifying ongoing supports as needed (medical and/or behavioral) in collaboration with appropriate personnel

· Complete the SIS, HRST, and ISP trainings by DBHDD

COMPETENCY REQUIRED

· Graduate from an accredited college

· Speak and write fluent English

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
